The fee for service is due in full at the time of your session. I accept all major credit cards and health savings account cards. You will be asked to store your payment card information via my practice’s secure portal.

I am an “out-of-network” provider and my office does not accept or file insurance. However, many PPO insurance plans will cover a percentage of my services as part of an “out-of-network benefit.” I can provide to you a comprehensive statement including all information required for you to submit a claim to your policy provider. If any additional information is required by your insurance coverage, such as reports for service authorization, those can also be provided if you request. We’ll talk before anything is completed or sent in so you won’t have any surprises. Your privacy decisions and your wishes are the most important factors.

You can file your insurance without cost directly with your insurance company, or you can use one of several online services which, for a fee, will help with out-of-network filing.
